- Life Insurance Policy at least six months old with Latest Receipt
- Valid Passport /OR
- Employer's certificate in case of Central Govt./State Govt./Local bodies supported
by attested copy of Identity Card.

- Educational Certificate /OR
- Birth certificate issued by Registrar Birth and Death showing the name of the applicant
/OR
- Life Insurance Policy at least six months old /OR
- Valid Passport /OR
- Employer's certificate in case of Central Govt./State Govt./Local bodies supported
by attested copy of Identity Card
- Certificate granted by registered medical practitioner, not below the rank of civil
surgeon, as to the age of the applicant.
- Driving License issued by RLA, UT. Chandigarh (for Date of Birth only)

Following documents accepted as proof of Address & Age supported by Affidavit attested
by Public Notary /Magistrate.

- Telephone Bill (BSNL Landline Only).
- Certificate issued by the Hostel Wardens in case of students residing in Hostels.
- Marriage Certificate issued by the Register of marriages, Chandigarh.
- Residence Certificate issued by the Chandigarh Administration.
- Rent Deed/ Rent agreement Registered with Sub-Registered of Chandigarh.
- Police Report from the concerned S.H.O.
- House Allotment Letter issued by Chandigarh Administration.
- Ownership Letter issued by Estate Office/Chandigarh Housing Board.
- Certificate of Registration of Firm (for RC only).
- Electricity Charges Bill.
- Sr. Citizen Identity Card issued by Social Welfare Department U.T., Chandigarh.
- Water Charges Bill.
- Identity Card issued to the Ex-serviceman, by the Zila Sanik Board.

- Certificate of age confirmation in case of illiterate person from Medical Officer
of Government Dispensary

Note : |
Learner License is valid for six months only, it can be renewed only once supported
by Medical certificate from M.B.B.S. Doctor in Form No.1 within 29 days from the
date of expiry.
|
|
|
Applicant can appeal for the test for the issue of Regular License after 30 days
of the issue of Learner License and before expiry of six months. |

Please bring original documents whose photocopies have been attached with the file.Passing
of Learner's License 9:00 A.M. to 1:00 P.M. at Counter No. 2 & 3 (for Men) in basement
and Counter No.10 (for Senior Citizen, Handicapped & Ladies only). |
