- FORM NO.1 (Medical certificate from MBBS Doctor)
with blood group
- FORM NO.9

1. Residence Proof : |
- Attested copy Voter card /OR
- Valid passport /OR
- LIC cover note at least six months old with latest receipt of payment /OR
- Employer certificate issued by the competent authority with I-Card in case of employee
of state or central govt. /OR
- If an applicant does not have any of the proof mentioned above he may attach copy
of Ration Card with affidavit.

by Public Notary /Magistrate. |

- Telephone Bill OR
- Electricity Charges Bill OR
- Water Charges Bill.
- Sr. Citizen Identity Card issued by Social Welfare Department U.T., Chandigarh OR
- Identity Card issued to the Ex-serviceman, by the Zila Sanik Board
